WELLNESS ON WHEELS (W.O.W.)
We collaborate with community organizations and service providers to deliver free mobile health screenings (e.g. blood sugar, blood pressure, cholesterol, prostate cancer) at their service locations and community events. Our goal is to improve the health outcomes for members of underserved BIPOC communities by providing monitoring supplies such as blood glucose meter kits and blood pressure monitors, referrals and assistance with applying for free/low-cost social assistance programs, educational materials and wellness workshops.
UNHOUSED PATIENT SERVICES (U.P.S.)
We provide street outreach and follow-up services for unhoused patients at discharge and prior to presenting to the Emergency Room for acute and chronic conditions. Our goal is to improve the health outcomes for this vulnerable population by addressing their needs holistically by providing:
-nutritious meals
-personal hygiene -products/toiletries
-weather appropriate clothing and footwear
-waterproof storage for personal belongings (duffle bags, backpacks, etc.)
-assistance with transportation to temporary housing/shelters
-referrals for assistance with applying for free/low-cost social assistance programs
-assistance filling prescriptions and obtaining medical screening supplies (e.g. blood glucose kits, BP monitors)
-assistance booking and providing transportation for follow-up care appointments
MOBILE PANTRY
We conduct street outreach and collaborate with community organizations and service providers at their service locations and community events to provide access to free clothing, houseware, school supplies, toiletries and groceries via our mobile van.
EAT TO LIVE – EXPRESS
We conduct street outreach and collaborate with community organizations and service providers at their service locations and community events to provide free, nutritious meals via bulk delivery and/or food truck distribution.
